Before Completing This Form, Please Ensure You Have The Following:

Scanned copies of 100-points of Identification:

• Australian Passport/Birth Certificate – if Overseas, Passport and visa/right to work in Australia.
• And an Australian Driver's License or any other Australian photo ID is acceptable 

Copy of current police check (NPC expires after three years) or NDISWC

Copies of relevant qualifications (related to the registration groups you have applied for)

Completed the ‘NDIS Worker Orientation Program’ (approximately 1.5hrs) and have the Certificate ready for upload

Sign up and register for this 30-minute eLearning module, "Supporting People to Stay Infection Free", with proof of completion (screenshot)

Confirm you have read through ‘COVID-19 Infection Control Training’ (30min)

Completed an NDIS Worker Screening in your applicable state
